Purpose

We describe the impact of a multifaceted program for decreasing ventilator-associated pneumonia (VAP) after implementing nine preventive measures, including selective oropharyngeal decontamination (SOD).

Methods

We compared VAP rates during an 8-month pre-intervention period, a 12-month intervention period, and an 11-month post-intervention period in a cohort of patients who received mechanical ventilation (MV) for?>?48 h. The primary objective was to assess the effect on first VAP occurrence, using a Cox cause-specific proportional hazards model. Secondary objectives included the impact on emergence of antimicrobial resistance, antibiotic consumption, duration of MV, and ICU mortality.

Results

Pre-intervention, intervention and post-intervention VAP rates were 24.0, 11.0 and 3.9 VAP episodes per 1000 ventilation-days, respectively. VAP rates decreased by 56% [hazard ratio (HR) 0.44, 95% CI 0.29–0.65; P?<?0.001] in the intervention and by 85% (HR 0.15, 95% CI 0.08–0.27; P?<?0.001) in the post-intervention periods. During the intervention period, VAP rates decreased by 42% (HR 0.58, 95% CI 0.38–0.87; P?<?0.001) after implementation of eight preventive measures without SOD, and by 70% after adding SOD (HR 0.30, 95% CI 0.13–0.72; P?<?0.001) compared to the pre-intervention period. The incidence density of intrinsically resistant bacteria (to colistin or tobramycin) did not increase. We documented a significant reduction of days of therapy per 1000 patient-days of broad-spectrum antibiotic used to treat lower respiratory tract infection (P?<?0.028), median duration of MV (from 7.1 to 6.4 days; P?<?0.003) and ICU mortality (from 16.2 to 13.5%; P?<?0.049) for patients ventilated?>?48 h between the pre- and post-intervention periods.

Conclusions

Our preventive program produced a sustained decrease in VAP incidence. SOD provides an additive value.

The development of children over time is one of the continuing aims of psychological inquiry. Many studies have concluded that children who grew up in dysfunctional households show inconsistent development across various developmental pathways. Previous studies have consistently concluded how different family factors, such as interparents relationship, parent–child relationships, and emotional relations in families, can explain the outcomes of child behavior problems. Therefore, this paper attempts to review the empirical findings of previous studies that assess the effects of marital relations on child behavioral development by reviewing the literature that supports the association between marital relations and behavioral problems of children. Twenty-seven studies were chosen for their outcomes. This review used online databases such as Google Scholar, Scopus, ProQuest, Psychological and Behavioral Science collection, SAGE full-text collection in psychology, and PsycINFO. The key words used were marital relation, marital conflict, marital satisfaction, child behavior problem, and offspring behavior adjustment. All articles hypothesized that marital relation affects child adjustment and behavior outcomes directly and indirectly. Approximately all the research samples were in the developmental stage (ages 3 to 12). Various methodologies were used in the studies, but some methodological defects in the literature in some way diminished the interpretation of their findings. Results revealed that children who come from families high in conflict are at risk of psychopathological developmental problems in their adolescent years. This review adds to a large collection of literature that highlights the damaging effects of marital conflict on children. Research limitations and suggestions for future studies are outlined. The study provides a concrete basis for further research on this topic considering that married couples and child behavior problems are varied. This review can benefit married and soon-to-be married couples by revealing perceptions into the experiences of others as regards marriage problem solving.  相似文献

Background:

Infection with Trichostrongylus spp. is common among human and herbivorous in most parts of Iran, especially in southern and northern areas. The aim of present study was to identify Trichostrongylus spp. among human population using excreted egg specimens, by the molecular method, in Mazandaran Province, northern Iran.

Methods:

Overall, 33 positive fecal specimens were randomly sampled and examined. PCR amplification of ITS2-rDNA region was performed on the isolated egg and then a restriction fragment length polymorphism (RFLP) profile was considered to discriminate of Trichostrongylus spp.

Results:

A total of 33 positive fecal specimens, 29(78.9%), 4(12.1%) were found T. colubriformis and T. axei respectively. Our data appear the molecular evidence of both human T. colubriformis and T. axei infections in North of Iran.

Conclusion:

T. colubriformis was the probable most common zoonotic species causing human trichostrongylosis infection in the area  相似文献

Background:

The trematodes of the genus Fasciola (the liver flukes) are among the well-known instances of food-borne parasites worldwide. Differentiation of Fasciola species is important because of their different transmission and epidemiological characteristics. The current study was undertaken to discriminate Fasciola species in the domestic ruminants of Urmia city, Iran.

Methods:

Adult flukes were isolated from the naturally infected livers of the slaughtered water buffaloes and sheep. The flukes were initially identified based on morphological and morphometric parameters. A 618-bp-long fragment of the 28SrRNA gene of Fasciola was amplified by polymerase chain reaction (PCR). The amplified fragment was digested by DraII or AvaII enzymes for a restriction fragment length polymorphism (RFLP) analysis and sequenced for the phylogenetic tree construction.

Results:

Based on the morphometric examination, the flukes belonged to F. hepatica, F. gigantica and an intermediate Fasciola form. The PCR-RFLP analysis was able to differentiate F. hepatica from F. gigantica. While the phylogenetic reconstruction justified, to some extent, the morphological diagnosis, it failed to segregate F. hepatica from F. gigantica identified in this and the previous studies.

Conclusion:

To resolve fully the problem of taxonomy and evolution in Fasciola species, employing a broad range of molecular and morphological approaches is necessary. This is crucial for epidemiological surveys and successful clinical management of their infection.  相似文献

Objectives

Widespread use of β-lactam antibiotics could cause resistance to this group of antibiotics in pathogenic bacteria through the production of the enzyme β-lactamases. The aim of this study is to determine the molecular detection of AmpC β-lactamases among clinical Escherichia coli isolated from Ilam hospitals in Ilam, Iran.

Methods

One hundred and twelve clinical isolates of E. coli were collected from hospitalized patients and were identified by biochemical tests. They were evaluated for extended spectrum beta-lactamases (ESBLs) production, and the positive strains were subjected to AmpC enzymes; for detection of AmpC cluster genes, multiplex polymerase chain reaction was applied.

Results

The analysis showed 62.5% of isolates were ESBLs positive and that five strains revealed the AmpC cluster genes. This is the first report of FOXM cluster genes in E. coli in Iran.

Conclusion

Based on our results, the prevalence of AmpC β-lactamases is increasing in Iran, which caused failure in antibiotic therapy. So, the current study recommended the revision of antibiotic policy in Iranian hospitals.  相似文献

Recently, nanosized metal-oxides have been extensively investigated for their ability to remove metal ions from aqueous media. However, the activity and capacity of these nanosized metal-oxides for removing metal ions decrease owing to their agglomeration in aqueous media. Herein, we synthesized a highly stable and magnetically separable rosin-biochar-coated (RBC) TiO2@C nanocomposite through a facile and environment-friendly wet chemical coating process, followed by a one-step heating route (pyrolysis) for efficient removal of Cr(vi) from aqueous solution. An array of techniques, namely, TEM, HRTEM, TEM-EDS, XRD, FTIR, VSM, BET and TGA, were used to characterize the prepared nanocomposite. The pyrolysis of rosin into biochar and the fabrication of Fe onto the RBC-TiO2@C nanocomposite were confirmed by FTIR and XRD examination, respectively. Moreover, TEM and HRTEM images and elemental mapping using TEM-EDS showed good dispersion of iron and carbon on the surface of the RBC-TiO2@C nanocomposite. Sorption of Cr(vi) ions on the surface of the RBC-TiO2@C nanocomposite was very fast and efficient, having a removal efficiency of ∼95% within the 1st minute of reaction. Furthermore, thermodynamic analysis showed negative values of Gibb''s free energy at all five temperatures, indicating that the adsorption of Cr(vi) ions on the RBC-TiO2@C nanocomposite was favorable and spontaneous. Conclusively, our results indicate that the RBC-TiO2@C nanocomposite can be used for efficient removal of Cr(vi) from aqueous media due to its novel synthesis and extraordinary adsorption efficacy during a short time period.

A smartphone-based microfluidic platform was developed for point-of-care (POC) detection using surface plasmon resonance (SPR) of gold nanoparticles (GNPs). The simultaneous colorimetric detection of trace arsenic and mercury ions (As3+ and Hg2+) was performed using a new image processing application (app). To achieve this goal, a microfluidic kit was fabricated using a polydimethylsiloxane (PDMS) substrate with the configuration of two separated sensing regions for the quantitative measurement of the color changes in GNPs to blue/gray. To fabricate the microfluidic kit, a Plexiglas mold was cut using a laser based on the model obtained from AutoCAD and Comsol outputs. The colorimetric signals originated from the formation of nanoparticle aggregates through the interaction of GNPs with dithiothreitol – 10,12-pentacosadiynoic acid (DTT-PCDA) and lysine (Lys) in the presence of As3+ and Hg2+ ions. This assembly exhibited the advantages of simplicity, low cost, and high portability along with a low volume of reagents and multiplex detection. Heavy Metals Detector (HMD), as a new app for the RGB reader, was programmed for an Android smartphone to quantify colorimetric analyses. Compared with traditional image processing, this app provided significant improvements in sensitivity, time of analysis, and simplicity because the color intensity is measured through a new normalization equation by converting RGB to an Integer system. As a simple, real-time, and portable analytical kit, the fabricated sensor could detect low concentrations of As3+ (710 to 1278 μg L−1) and Hg2+ (10.77 to 53.86 μg L−1) ions in water samples at ambient conditions.

It is believed that neuropathic pain results from aberrant neuronal discharges although some evidence suggests that the activation of glia cells contributes to pain after an injury to the nervous system. This study aimed to evaluate the role of microglial activation on the hyper‐responsiveness of wide dynamic range neurons (WDR) and Toll‐like receptor 4 (TLR4) expressions in a chronic constriction injury (CCI) model of neuropathic pain in rats. Adult male Wistar rats (230 ± 30 g) underwent surgery for induction of CCI neuropathy. Six days after surgery, administration of minocycline (10, 20, and 40 mg/kg, i.p.) was initiated and continued until day 14. After administration of the last dose of minocycline or saline, a behavioral test was conducted, then animals were sacrificed and lumbar segments of the spinal cord were collected for Western blot analysis of TLR4 expression. The electrophysiological properties of WDR neurons were investigated by single unit recordings in separate groups. The findings showed that after CCI, in parallel with thermal hyperalgesia, the expression of TLR4 in the spinal cord and the evoked response of the WDR neurons to electrical, mechanical, and thermal stimulation significantly increased. Post‐injury administration of minocycline effectively decreased thermal hyperalgesia, TLR4 expression, and hyper‐responsiveness of WDR neurons in CCI rats. The results of this study indicate that post‐injury, repeated administration of minocycline attenuated neuropathic pain by suppressing microglia activation and reducing WDR neuron hyper‐responsiveness. This study confirms that post‐injury modulation of microglial activity is a new strategy for treating neuropathic pain.  相似文献
